Purpose of the Form
To register and document affected women in Gaza within the targeted categories, in order to provide humanitarian, social, and financial support to them and their families. The program builds a comprehensive database that helps identify the most vulnerable women and ensures fair and organized assistance delivery, with a registration status inquiry option.

• Martyrs’ widows.
• Prisoners’ wives.
• Injured persons’ wives.
• Missing persons’ wives.
• Divorced women.
Required Information in the Registration Form
Section One: Personal, Husband, Housing, and Documents
• Full name, ID number, date of birth.
• Marital status and education level.
• Contact details (mobile, alternative number, WhatsApp).
• Husband’s information (name, ID, date of death or disappearance).
• Current housing details and residence type.
Section Two: Family, Financial, Health, Assistance, and Needs
• Health conditions and care needs.
• Income source and monthly income.
• Bank or wallet details.
• Previous assistance and projects.
• Selection of family needs.
Section Three: Children Data
• Add each child separately with full personal, health, and education details.
Available Support Types
• Monthly sponsorship.
• Educational support.
• Medical and health support.
• Shelter assistance.
• Psychosocial support.
• Food assistance.

All required fields must be completed accurately.
The form is for women only within the specified categories.
Bank or wallet details must be entered correctly.
Multiple needs may be selected.
There is no limit on the number of children added.
Supporting documents are required depending on the case.
