• Marital status.
• Status of the household breadwinner.
• Family size.
• Children’s information.
• Original place of residence.
• Whether the family is displaced from their original home.
• Health status.
• Educational level and skills.
• Copy of ID with the full attachment.
• Medical report (if available).
• Additional notes.
