Purpose of the Form
To enable parents and caregivers in Gaza to register their children for the Juthoor Nutrition Program, collect accurate household data, conduct comprehensive nutritional assessments, and determine eligibility for nutritional support according to approved program criteria.
Required Information in the Registration Form
• Head of household full name.
• Head of household ID number.
• Mother’s full name.
• Mother’s ID number.
• Current place of residence (e.g., Beit Lahia, Beit Hanoun, Shuja’iyya, etc.).
• Mobile phone number.
• Alternative mobile number (if available).
• Mother’s status (pregnant, breastfeeding, accompanying a child).
• Children’s details for ages 6 months to 12 years.
Important Notes
Children must be residents of North Gaza or Gaza City.
Eligible children must be between 6 months and 12 years old to receive nutrition services.
Children already registered and followed as malnutrition cases within the foundation’s centers are not eligible for re-registration.
Pregnant and breastfeeding women may register, provided proof of pregnancy is presented during assessment.
No child or mother will be received without prior online registration and SMS appointment confirmation.
All information must be entered accurately; incorrect data will result in exclusion, and duplicate registrations for the same family will delay eligibility automatically.
Children who do not meet the above conditions will not be received.
Duplicate submissions may cause loss of early notification priority; please register only once per family.
A valid and active mobile phone number is essential, as appointment details will be sent via SMS.
