Purpose of the Form
The form aims to support operating bakeries by providing the gas required to run production lines, ensuring continuous bread production and meeting community needs, with priority given to bakeries serving a large number of residents and displaced families in surrounding areas.
Required Information in the Registration Form
Applicants are required to provide the following details:
Bakery owner’s name
Bakery name
Geographic location of the bakery
License number
A copy of the valid bakery license
Application Requirements
To be eligible, the bakery must meet the following conditions:
Hold a valid operating license
Have all essential production equipment except gas
Commit to official bread prices and approved loaf weight
Use gas exclusively for production purposes
Serve the largest possible number of displaced people within its geographic area

Submitting the form does not guarantee gas approval, as all applications are subject to evaluation based on official criteria.
Applicants must specify the current fuel type used and the monthly gas quantity required.
